fa تجزیه و تحلیل عوامل ریشه‌ای حوادث منجر به پیامدهای شدید در شرکت نفت لیان با استفاده از تکنیک تریپود بتا ایمنی و حوادث ناشی از کار Safety and occupational accidents پژوهشي Research <strong><span style="font-family:B Nazanin;"><span style="font-size:10.0pt;">مقدمه:</span></span></strong> <span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">بدون شک اوّلین گام در راستای پیشگیری از حوادث، آنالیز آنها به منظور شناسایی علل ریشه‌ای است. در این مطالعه عوامل ریشه‌ای حوادث منجر به پیامدهای شدید با استفاده از تکنیک تریپود بتا در شرکت نفت لیان تجزیه و تحلیل شده است.</span></span><br> <strong><span style="font-family:B Nazanin;"><span style="font-size:10.0pt;">روش بررسی:</span></span></strong> <span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">این مطالعه به روش توصیفی تحلیلی است. در این پژوهش تجزیه و تحلیل حوادث در دو واحد بشکه سازی و بشکه پرکنی شرکت پالایش و تجارت نفت لیان در سال 1399 انجام شده است. روش تریپود بتا (</span></span><span dir="LTR"><span style="font-size:10.0pt;">Tripod-beta</span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">) یکی از روش‌های ریشه‌یابی حوادث است، که به دلیل نگرش خاص به عنصر خطای انسانی به طور وسیع در صنایع نفتی استفاده می‌شود. با استفاده از نرم‌افزار </span></span><span dir="LTR"><span style="font-size:10.0pt;">investigator</span></span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">، نمودار تریپود بتا مربوط به سه حادثۀ شکستگی و نقص عضو رسم شد و پس از ثبت و شناخت علل و موانع مربوط، اقدامات اصلاحی برای جلوگیری از بروز حوادث مشابه بیان شد. اساس کنترل حوادث، ایجاد سیستم‌های کنترلی و دفاعی مناسب به نحوی است که از تلاقی و برهم کنش عامل خطر و عامل هدف جلوگیری شود. </span></span><br> <strong><span style="font-family:B Nazanin;"><span style="font-size:10.0pt;">نتایج:</span></span></strong> <span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">نتایج این مطالعه نشان می‌دهد که عدم رعایت اصول ایمنی به دلیل کاهش آگاهی کارکنان و کمبود آموزش‌های دوره‌ای بلند مدت و کوتاه مدت از عوامل ریشه‌ای حوادث مطالعه شده و بالاترین تأثیر را در رویدادهای مزبور داشته است. 45 درصد موارد مربوط به این علت حادثه بوده است. سایر موارد بعد از آموزش تقریباً به یک اندازه در رخداد رویدادها تأثیرگذار بوده‌اند. هر کدام از موارد 11 درصد علل مربوط به حوادث را به خود اختصاص داده‌اند.</span></span><br> <strong><span style="font-family:B Nazanin;"><span style="font-size:10.0pt;">نتیجه‌گیری:</span></span></strong> <span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">تکنیک تریپود بتا در حال حاضر یکی از بهترین روش‌های ریشه‌یابی حوادث است. همچنین نتایج بدست آمده حاکی از آن است که انجام اقدامات مؤثر در فاز زمانی نرمال یعنی بکارگیری افراد با مهارت استعداد بالاتر و انجام آموزش‌های بدو استخدام و انجام دیدگاه بالادستی یعنی هماهنگی اپراتور با سخت افزار و روش‌های اجرایی در فاز ایده و تفکر و طراحی به میزان زیادی از رخداد حوادث جلوگیری خواهد کرد.</span></span><br> <span style="font-family:B Nazanin;"><span style="font-size:12.0pt;"></span></span> <em><strong>Introduction:</strong></em>&nbsp;Undoubtedly, the first step in controlling accidents is to analyze them to identify the root causes. In this study, the root causes of accidents leading to severe consequences have been analyzed using the beta tripod technique in Lian Oil Company.<br> <em><strong>Materials and Methods:</strong></em>&nbsp;The research method is descriptive-analytical. In this research, the events of 2020 in two barrel-making and barrel filling units of Lian Oil Refining and Trade Company have been investigated. The tripod-beta method is one of the methods of tracing the roots of accidents, which is widely used in the oil industry because of its special attitude towards the element of human error. Using investigator software, a tripod beta diagram related to three fracture and disability accidents was drawn and after recording and identifying the relevant causes and obstacles, corrective measures were taken to prevent similar accidents. The basis of accident control is the creation of appropriate control and defense systems in such a way as to prevent the interconnection and interaction of the risk factor and the target factor.<br> <em><strong>Results:</strong></em>&nbsp;The results of this study show that non-compliance with safety principles due to reduced staff awareness and lack of long-term and short-term training are the root causes of accidents studied and had the highest impact on these events. 45% of cases were related to the cause of the accident. Other cases after training have had almost the same effect on the occurrence of events. Each of the cases accounted for 11% of the causes of the accidents.&nbsp;<br> <em><strong>Conclusion:</strong></em>&nbsp;Beta tripod technique is currently one of the best methods for tracing the roots of accidents.
