A System-Level Investigation of Human and Organizational Failures in Mining Control Rooms

Abstract
Introduction: In high-risk industries, particularly mining, accidents frequently arise from complex interactions between human elements and organizational structures. The control room, as the operational command center, plays a critical role in maintaining safety and ensuring the continuity of production processes. Therefore, identifying and analyzing potential errors in this environment is of paramount importance.
Methods: In this study, the Human Factors Analysis and Classification System was applied to systematically identify and categorize factors contributing to errors. In addition, fuzzy logic was employed to quantitatively estimate the likelihood of error occurrence. Data were collected through structured questionnaires and expert judgments, and analyzed using linguistic variables and fuzzy membership functions.
Results: The findings indicated that the highest probability of error was associated with organizational factors (approximately 99.8%), highlighting the decisive role of policies, resource management, and organizational climate in error formation. Unsafe supervision ranked second with a probability of 54.1%, reflecting deficiencies in monitoring, control, and timely corrective actions. Furthermore, preconditions for unsafe acts (36.1%) and unsafe acts themselves (24.8%) were identified as other significant contributors.
Conclusion: The hierarchical analysis based on study revealed that errors exhibit a chain-like nature, with many root causes embedded in higher system levels. Disruptions at the organizational level can propagate downward in a cascading manner, significantly increasing the likelihood of adverse events. These findings underscore the importance of adopting preventive strategies at the macro-management level.
